DaVita Announces New Value-Based Partnership with Humana to Expand Care for Patients with Chronic Kidney Disease
DaVita has announced a new value-based care agreement focused on providing comprehensive support for Humana Medicare Advantage members suffering from chronic kidney disease (CKD) stages 3B–5. This initiative aims to assist more than 10,000 patients in delaying or preventing the need for dialysis and reducing hospitalizations.
This partnership, which launched on July 1, expands the existing collaboration between DaVita and Humana by addressing CKD at an earlier stage, where intervention is most effective. Through its DaVita Integrated Kidney Care (DaVita IKC) program, the organization is committed to delivering coordinated care that accounts for the interconnected aspects of cardiovascular, kidney, and metabolic health.
Dr. Sonia Samagh, national medical director for DaVita IKC, emphasized the importance of early intervention in kidney care, stating, "We are in a therapeutic revolution across kidney care where upstream interventions are helping to reshape patient outcomes." The partnership aims to provide advanced care that not only treats patients holistically but also preserves kidney function while managing other health conditions.
The program is particularly focused on engaging patients at a critical juncture in their disease progression, where kidney decline often accelerates but remains undiagnosed or undertreated. This approach is designed to stabilize kidney function and slow disease progression through consistent monitoring and intervention.
In cases where patients cannot avoid kidney failure, the partnership also emphasizes the importance of advanced care planning, early education on treatment options, and support from interdisciplinary care teams. Dr. Samagh noted the systemic nature of kidney health, stating, "To truly care for the kidneys, we must uncover and address those underlying health issues," highlighting the model's aim to enhance patient quality of life.
DaVita, a healthcare provider recognized for its focus on improving patient care and clinical quality over the past 25 years, has served approximately 298,500 patients across its global network, which includes over 3,200 outpatient dialysis centers.
